What Ages Do You Accept?
Newborn through five or six years old (depending on kindergarten eligibility).
How Many Children are in Care at Once?
Care is capped at a maximum of 5 children at once for safety and personal attention.
Are Meals Included in The Tuition? If Not, Do You Offer a Meal Plan for an Additional Cost?
Parents send meals/snacks from home at no additional charge.

1 Day/Week is $26 Hourly. Perfect for running errands, emergencies or date night.
Note: Tuition is paid in advance each Friday for the upcoming week.
What Forms of Payment Do You Accept?
We accept cash, personal checks, debit/credit cards, and money orders.
Returned checks incur a $30 fee; after two returned checks, cash only.
Do You Accept Child Care Assistance or Subsidies?
Yes. We accept programs such as NORWESCAP. Please Note: Families are responsible for any amount not covered.
How Do Early Drop-offs and Late Pick-ups Work?
Early drop-off (before 7am) requires 48 hours advance approval and is billed at $20 per hour; without prior notice, the rate is $30 per hour.
Late pick-up: Is billed at $1 per minute after closing. Please note that after 3 late pick-ups, the fee increases to $25 for the first 15 minutes, plus $1 per minute after that.
Holidays: All children MUST be picked-up by 3:00 p.m. If a child is not picked up by the designated time, a late fee of $45 will be applied, for the first 15 minutes, and $2 for every minute thereafter due to the holiday.

What is Your Illness Policy and When Can my Child Return?
For the safety of everyone, J-Max & Friends requires to keep your child home for symptoms like:
*Fever (100.4°F+)
*Vomiting/diarrhea within the last 24 hours
*Persistent cough/difficulty breathing,
*Rash that has not been diagnosed or may look like measles
*Eye discharge or pink eye (conjunctivitis)
*Head lice or nits
*Any contagious illness (e.g., flu, COVID-19, strep throat).
*Fatigue or irritability that prevents participation in normal activities
Children may return when symptom-free for 24 hours without medication or with a doctor’s note.

What Health Records are Required?
A current physical, up-to-date immunizations, and the
Universal Child Health Record (UCHR) are required before enrollment and it MUST be updated annually.
What Should We Bring Each Day?
Please label everything. Typical items include:
*Extra clothes
*Diapers/pull-ups
*Wipes
*Diaper cream (if applicable)
*Formular/breastmilk (if applicable)
*Blanket for nap
*Lunch/snacks
*Baby bottles/bibs/pacifiers (infants)
*Seasonal outerwear
What Hygiene and Care Routines Do You Follow for the Children?
With parent consent, staff may:
*Bathe child after toileting accidents
*Apply body lotion/diaper cream
*Comb/style hair
*Gently trim nails to prevent scratching and promote cleanliness.
Can You Administer Medication?
Yes—with a signed Medication Authorization Form. Medication must be in its
original labeled container with dosage instructions.
Do You Take Photos or Videos?
Only with your explicit Photo/Video Consent. If granted, media may be used for school communications
(website, social media, press) in line with our privacy practices.
Transportation and off-site Activities
Off-site trips and transportation require your written authorization. Participation is always optional and supervised.
What is Your Withdrawal/Termination Policy?
Either party may terminate this contract with two weeks’ written notice. Parents are still required
to pay the full tuition amount for the final two weeks, regardless of whether the child attends
during that time or not. The provider reserves the right to terminate care immediately if payment
is not made promptly.
Do You Have an Expulsion Policy?
While uncommon, expulsion/suspension may occur for:
*Safety
*Repeated nonpayment
*Missing immunization records
*Persistent late pick-ups
*Serious behavioral concerns
*Parental misconduct
